Physician Associates online appointment request application is for ROUTINE appointments only. For all other appointment requests, please call your physician's office. Physician Associates will respond to your appointment request within 2 business days. Physician Associates will contact you via secure email or by telephone to approve, deny or modify an appointment request.

Physician Associates DOES NOT provide emergency health care assistance via its web site.

If you are having an emergency, please call 911 immediately.

Prescription Renewals

Physician Associates reserves the right to deny any prescription refill requests. Requests for refills of narcotics will not be accepted or approved using this online service. All requests for refills of controlled substances require an appointment with your physician.

You can expect your prescription approval or denial notification via secure email within 24 hours of your request, during normal business hours.
